Insurances Accepted

I currently am in network with Pacific Source, Moda Health, and Regence / Blue Cross Blue Shield.

I am currently moving out of network with Cigna and Healthnet effective 11/1/2025, and Providence effective 1/1/2026 and will no longer be able to bill those insurances after those dates.

If I am out of network with your insurance, I am able to provide you with a superbill for cash pay visits to submit to your insurance for reimbursement if you have Out Of Network benefits. Payment would be required by the patient at the time of the visit.

Please note: I am not able to bill Oregon Health Plan (OHP/CareOregon), even when administered through the above insurance companies.

Also, I am only able to bill select Medicare Advantage plans administered through the above Insurance companies. I cannot bill many Medicare plans, which includes standard Medicare Part B. Please double check with your plan to ensure I am in network.
